Military Service Verification Form

    This form is intended for veterans, military service members, and their spouses who are seeking an application fee waiver on a graduate admission application. 

    Please Read: Before completing this form, read the policy on application fee waivers. This form refers only to the LSU graduate application fee, not to any other university, department, or program fees that may apply. Application fee waivers are not available for international students or late applications. Application fee waiver requests must be submitted at least four weeks prior to the program’s application deadline.

    Application fee waiver requests must be submitted at least four weeks prior to the program’s application deadline date, in order to provide enough time to evaluate the application and determine eligibility. Application fee waivers will not be granted if the application deadline has passed. The program to which you are applying may require additional fees, which are not covered by this policy.

    Fill out the Military Service Request Form, and append the documents listed on the form applicable to you. (Note: application fee waiver requests will not be considered without the required documentation). Final approval for the requested waiver is made by the LSU Graduate School.
